Starting in 2012, Instructors under AED Institute will be required to use the new forms while conducting certifications and other information classes. Please review the instructions before submitting your forms to reduce any delay in processing your classes.
QUICK LIST – SUBMIT THE FOLLOWING FOR EACH CLASS:
1. COURSE ROSTER. Be sure ALL applicable fields are filled out.
2. SIGN IN SHEET. Submit original & TYPED list of names if certification cards are being issued.
3. EVALUATION SUMMARY. Submit one per class. Be sure to include comments.
