COURSE ROSTER DOWNLOAD AND INSTRUCTIONS

AED Institute Instructors: Starting in 2012, you are required to use the following forms in place of AHA standard forms when you are leading a class for AED Institute. Please note that your classes will not be processed until proper paperwork is completed and submitted. Thank you for your cooperation and for being part of our team! INSTRUCTOR CLASS FORM CHECK LIST

1. AED Institute 2012 Course Roster – Fill out ALL applicable fields.

Note: For the Training Site section – unless you are training for a site under AED Institute, leave this blank. Fill out Course Location – where the class was held.

2. AED Institute 2012 Sign In Sheet – Be sure to submit original and TYPED list with your roster.

IMPORTANT: AED Institute will only replace cards with names that differ from those submitted on TYPED Sign In sheets. Any other misspelled cards will cost $5.50 to replace.

3. Evaluation Summary.

It is the Instructor’s responsibility to submit a completed, consolidated Evaluation Summary with all courses. These Summary’s should include any submitted comments by students. Evaluations are there to help all Instructors of all levels to learn how to be more effective in their teaching methods.

PDF DOWNLOADS

INSTRUCTORS: PDF forms are NOT fillable. These can be printed and manually filled out. You can either scan the form, save as PDF and email to info@aedinstitute.com OR mail to our main office. Please note that a typed Sign In sheet is required with all certification classes but can be submitted manually in an email.

Note: The field “AED Institute Training Site Name:” can be left blank unless you are part of a Training Site under AED Institute. Otherwise, just fill in Course Location.

  • A typed list of names is required for all certification cards. You don’t have to use the formal Sign In Sheet, but be sure a typed list and originals are submitted with your roster and other paper work.

INSTRUCTORS: If you prefer to use a Word Document to type out your rosters and sign in sheets, use these (.doc) downloads. If you are submitting paperwork through email, please convert to PDF before sending if possible.

REQUIREMENTS: Microsoft Word 97 or higher.

  • A TYPED list of names is required for ALL CERTIFICATION CARDS. You don’t have to use the formal Sign In Sheet, but be sure a typed list (email okay!) and originals are submitted with your roster and other paper work. You do not need to submit a typed list for classes that do not require cards.
